What is the health plan?
The health plan is a personalized guide designed to help your client improve their health. It includes both individual and group protocols tailored to their needs. Clients can view the plan on the Welle app, purchase recommended products, and track their progress—all in one place.
What tools are available to help you?
To make building the health plan easier, the system provides various personalized data and suggestions, including:
- Client info: Basic details like name, age, sex, membership status, and a quick link to their profile.
- Health intake forms: The client’s latest medical intake updates.
- Biomarker results: Current and historical biomarker data to assess health trends.
- Weekly progress: A snapshot of how the client has been completing their plan, making it easier to see what’s working and where improvements are needed.
- Previous notes: Summaries of SOAP notes from earlier sessions to help you understand their journey so far.
How does the system support you?
- Protocol suggestions: The system suggests protocols based on factors like age, sex, biomarkers, and health profile. These give you a starting point to create an effective plan.
- AI-generated content: To save you time, the system drafts SOAP notes, follow-up summaries, and communication templates that you can refine as needed.
Let’s break this process into three simple steps to guide you:
Step 1: Complete your telehealth session
This step is all about gathering the right information to build a personalized plan for your client.
1. Review the client’s details
Before your telehealth session, take a moment to review:
- The client’s health intake form and biomarker results to understand their health history.
- Weekly progress to spot trends or areas needing extra attention.
- Product recommendations: The system pre-selects products that match the client’s biomarkers and health profile. These can be helpful to introduce during the session.
2. Focus on the client’s goals and needs
During the telehealth session, your goal is to understand the client’s priorities.
- Ask about their preferences and clarify any information from their health records.
- Use this time to set the foundation for a plan that aligns with their goals.
- Introduce products early: If you plan to recommend supplements or medications, explain their benefits during the session. This helps ensure the client feels comfortable and motivated to follow the plan.
3. Wrap up the session with SOAP notes
- After the session, click finish to finalize the information you’ve gathered.
- The AI will generate the subjective and objective parts of the SOAP note for you. Review and make any edits to ensure accuracy.
Once this step is complete, you’ll have everything you need to start customizing the client’s health plan in Step 2.
Step 2: Customize and finalize protocols
Now it’s time to build the actual health plan, making sure it’s tailored to the client’s specific needs and preferences.
1. Understand the system’s protocol suggestions
The system suggests protocols based on the client’s profile. Here’s how it works:
- Age and sex: Protocols are filtered by age range and categorized for male, female, or unisex.
- Biomarkers: The client’s biomarker results are compared to protocol triggers. Matching results are highlighted, while non-matching ones are left blank.
- Health profile: The client’s intake form is analyzed to find conditions relevant to the protocols.
Keep in mind: Protocol groups are always prioritized in suggestion logic. Some protocols might appear without specific criteria like age or biomarkers if they fit other aspects of the client’s profile.
2. Review and adjust the suggested protocols
Take a closer look at the protocols recommended by the system. You’ll see:
- Biomarker matches: Highlighted in the biomarkers range column.
- Duration: Start and end dates for each protocol. Ideally, protocols should fit within the 90-day plan cycle.
- Left-over days:
- Protocols carried over from a previous plan will show remaining time.
- Protocols not started before are labeled Not Started.
- Completed protocols will show 0 days remaining.
- New protocols are marked New.
These info helps you to make decisions:
- Renew ongoing or completed protocols still relevant to the client’s goals (protocol renewal allow the accompanied product to be re-purchased).
- Remove redundant or unsuitable protocols based on client preferences.
- Add new protocols to address emerging needs.
3. Make sure the plan matches the client’s goals
Go back to the discussion from Step 1 and ensure the plan reflects what the client wants to achieve.
- Adjust protocols or their durations to make them practical and easy to follow.
- Focus on creating a plan that feels achievable and meaningful to the client.
4. Get ready for Step 3
Once you’ve reviewed and customized the protocols:
- Double-check that the plan is well-balanced and meets the client’s health goals.
- You’re now ready to finalize and activate the plan in Step 3.
Step 3: Finalize and activate the health plan
This is where everything comes together.
1. Review the entire health plan
Make sure the protocols you’ve selected—whether renewed, new, or ongoing—align with the client’s goals and timeline.
- Check that start and end dates are accurate, especially for any protocols carried over from a previous plan.
2. Confirm the care bag
The care bag includes all products linked to the protocols in the plan.
- Make sure everything is included for the client to purchase or subscribe to.
- If there are pharmaceuticals, the system will prompt you to sign an e-prescription.
3. Summarize the plan for the client
The follow-up letter is a great way to explain the health plan to the client.
- Highlight how the protocols address their specific goals.
- Point out important actions, like products to buy or start dates for protocols.
- Encourage the client to share feedback or discuss adjustments in future sessions.
4. Send the plan for activation
When you’re satisfied with the plan, click Send to client. This activates the new plan and replaces any previous versions. Once the plan is activated, all its details are stored in the system. This allows you to review progress, revisit decisions, and track the client’s health journey over time.
By completing Step 3, you’ve set the client up for success with a clear, actionable health plan!
